What is Goitre (Thyroid Enlargement)
Goitre simply means enlargement of the thyroid gland, which appears as a swelling in the front of the neck.
It’s not always due to overactivity or underactivity — sometimes the thyroid just grows in size.
Types of Goitre
1. Simple or Non-toxic Goitre:
- Thyroid is enlarged but functions normally (normal hormone levels).
- Commonly due to iodine deficiency.
2. Toxic Goitre:
- Thyroid is enlarged and overactive (producing too much hormone).
- Example: Graves’ disease, toxic multinodular goitre, etc.
3. Colloid or Multinodular Goitre:
- Gland becomes lumpy or nodular.
- Usually seen in long-standing simple goitre.
4. Endemic Goitre:
- Occurs in areas where iodine deficiency is common (mountainous regions, inland villages).
Causes
- Iodine deficiency (most common cause)
- Autoimmune diseases (like Hashimoto’s thyroiditis or Graves’ disease)
- Hormonal imbalance
- Drugs like lithium or amiodarone
- Congenital defect in thyroid hormone synthesis
Symptoms
- Visible swelling in front of the neck

- Voice may become hoarse
- In toxic goitre → symptoms of hyperthyroidism (weight loss, tremor, palpitations)

Investigations
- Thyroid profile test (T3, T4, TSH)
- Ultrasound of thyroid
- Thyroid scan (for nodules or functional status)
- FNAC if nodule suspected for malignancy
