What is Post-Radiation Thyroiditis

Post-radiation thyroiditis is a condition in which the thyroid gland becomes inflamed or damaged after radiation therapy.
This usually happens when a person receives radiation to the neck or upper chest — for example, during treatment for cancers of the throat, lymph nodes, or breast — or after radioactive iodine therapy used for hyperthyroidism.

The thyroid gland is quite sensitive to radiation. So, when it gets exposed, the cells of the gland can get inflamed or destroyed, leading to temporary or permanent thyroid dysfunction — either hyperthyroidism (overactive phase) or hypothyroidism (underactive phase).

Causes

1. External Beam Radiation:

  • Used in cancers of neck, head, larynx, or breast.
  • Damages thyroid cells and reduces hormone production.

2. Radioactive Iodine Therapy (RAI):

  • Given for treating overactive thyroid (Graves’ disease or toxic nodules).

Pathophysiology

Radiation leads to inflammation in the thyroid tissue (thyroiditis).
Initially, the damaged cells release stored thyroid hormones → causing temporary hyperthyroidism (thyrotoxicosis).
Later, when the gland becomes fibrosed or scarred, it loses its ability to produce hormones → leading to hypothyroidism.

So, it’s often a biphasic condition:

  •  Phase 1 – Temporary overactive thyroid
  •  Phase 2 – Permanent underactive thyroid

Symptoms

Early (Hyperthyroid) Phase:

  • Nervousness, restlessness
  • Palpitations, tremors
  • Sweating and heat intolerance
  • Weight loss despite good appetite

 Later (Hypothyroid) Phase:

  • Fatigue, sleepiness
  • Weight gain
  • Cold intolerance

Sometimes, mild neck pain or tenderness is felt in the thyroid region.

Diagnosis

  • Early phase: High T3/T4, low TSH
  • Later phase: Low T3/T4, high TSH
  • Anti-thyroid antibodies: May or may not be positive.
  • Ultrasound: May show heterogeneous or inflamed thyroid tissue.
