Collecting Duct Carcinoma (CDC)

What is CDC

Also called Bellini Duct Carcinoma.

  • A very rare subtype of renal cell carcinoma (<2% of RCC cases).
  • Originates from the collecting ducts of the kidney medulla (deep part of kidney).
  • Known for being highly aggressive → spreads early to lymph nodes, lungs, liver, and bones.

Risk Factors

Exact cause is unknown.

More common in:

  • Young to middle-aged adults (20–40 years) compared to other RCCs.
  • Males > females.
  • No strong lifestyle link, but genetic instability is suspected.

Symptoms

CDC often presents late because of its aggressive nature:

  • Persistent hematuria (blood in urine).
  • Severe flank or abdominal pain.
  • Abdominal or flank mass.
  • Fatigue, weight loss, fever.
  • Advanced cases → bone pain, cough, breathing issues (due to metastasis).

Complications

  • Rapid metastasis → lungs, bones, liver.
  • Renal failure if both kidneys or collecting ducts involved.
  • Very poor response to usual RCC therapies.

Diagnosis

Imaging (USG, CT, MRI) → irregular medullary mass, often infiltrative.

Biopsy + histology:

  • Tubulopapillary architecture.
  • Desmoplastic stroma (dense fibrous tissue).
  • Immunohistochemistry → helps differentiate from urothelial carcinoma & medullary carcinoma.

Conventional Treatment

  • Unfortunately, CDC is resistant to standard RCC therapies.
  • Radical nephrectomy (complete kidney removal) → primary approach if localized.
  • Chemotherapy (platinum-based like Cisplatin + Gemcitabine).
  • Targeted therapy & immunotherapy → tried, but outcomes still poor.
  • Clinical trials are often recommended.

Prognosis

  • Very poor prognosis compared to other RCCs.
  • 5-year survival rate: <10% in advanced cases.
